Which are the Symptoms of Nausea?

Symptoms are frequently difficult for people to describe. Nausea symptoms are not painful but very uncomfortable feelings that are felt in the chest, upper abdomen, or back of the throat. It can also be associated with:

  • Headache
  • Fever
  • Gas
  • Vomiting
  • Dizziness
  • Lightheadedness
  • Diarrhea
  • Abdominal pain, and
  • The general feeling of being sick to one’s stomach.

Other causes of nausea include side effects from medications, pain, pregnancy and morning sickness, seasickness, viral infection, alcohol toxicity, and disease of the brain, esophagus, stomach, gallbladder, liver, pancreas, and intestine.

What causes Nausea?
 

Gastrointestinal infections and food poisoning are the two most common causes of acute nausea and vomiting. Side effects of medications and pregnancy are also relatively frequent. There are many causes of chronic nausea. Nausea and vomiting remain undiagnosed in 10% of the cases.

Aside from morning sickness, there are no sex differences in complaints of nausea. After childhood, doctor consultations decrease steadily with age. Only a fraction of one percent of doctor visits by those over 65 is due to nausea.

Gastrointestinal

Gastrointestinal infection is one of the most common causes of acute nausea and vomiting. Chronic nausea may be the presentation of many gastrointestinal disorders, occasionally as the major symptom, such as:

  • Gastroesophageal reflux disease
  • Functional dyspepsia
  • Gastritis
  • Biliary reflux
  • Gastroparesis
  • Peptic ulcer
  • Celiac disease
  • Non-celiac gluten sensitivity
  • Crohn’s disease
  • Hepatitis
  • Upper gastrointestinal malignancy, and
  • Pancreatic cancer. 

Uncomplicated Helicobacter pylori infection does not cause chronic nausea.

Food poisoning

Food poisoning usually causes an abrupt onset of nausea and vomiting one to six hours after ingestion of contaminated food and lasts for one to two days. It is due to toxins produced by bacteria in food.

Nausea due to Medications

Many medications can potentially cause nausea. Some of the most frequently associated include cytotoxic chemotherapy regimens for cancer and other diseases and general anesthetic agents. An old cure for migraine, ergotamine, is well known to cause devastating nausea in some patients; a person using it for the first time will be prescribed an antiemetic for relief if needed.

Pregnancy Nausea

Nausea or “morning sickness” is common during early pregnancy but may occasionally continue into the second and third trimesters. In the first trimester, nearly 80% of women have some degree of nausea. 

Disequilibrium

A number of conditions involving balance such as motion sickness and vertigo can lead to nausea and vomiting.

Gynecologic

Dysmenorrhea can cause nausea.

Psychiatric

Nausea may be caused by depression, anxiety disorders, and eating disorders.

Potentially serious

These conditions include:

  • Pancreatitis
  • Small bowel obstruction
  • Appendicitis
  • Cholecystitis
  • Hepatitis
  • Addisonian crisis
  • Diabetic ketoacidosis
  • Increased intracranial pressure
  • Spontaneous intracranial hypotension
  • Brain tumors
  • Meningitis
  • Heart attack
  • Carbon monoxide poisoning, and many others.

Diagnosis

Physical exam

It is important to watch out for signs of dehydration, such as orthostatic hypotension and loss of skin turgor. Auscultation of the abdomen can produce several clues to the cause of nausea and vomiting. A high-pitched tinkling sound indicates possible bowel obstruction, while a splashing “succussion” sound is more indicative of gastric outlet obstruction.

Eliciting pain on the abdominal exam when pressing on the patient may indicate an inflammatory process. Signs such as papilledema, visual field losses, or focal neurological deficits are red flag signs for elevated intracranial pressure.

Diagnostic testing

When history and physical exam are not enough to determine the cause of nausea and vomiting, certain diagnostic tests may prove useful. A chemistry panel would be useful for electrolyte and metabolic abnormalities. Liver function tests and lipase would identify pancreaticobiliary diseases. 

Abdominal X-rays showing air-fluid levels indicate bowel obstruction, while an X-ray showing air-filled bowel loops are more indicative of ileus. More advanced imaging and procedures may be necessary, such as a CT scan, upper endoscopy, colonoscopy, barium enema, or MRI. 

Nausea Pathophysiology

Research on nausea and vomiting has relied on using animal models to mimic the anatomy and neuropharmacologic features of the human body. The physiologic mechanism of nausea is a complex process that has yet to be fully elucidated. There are four general pathways that are activated by specific triggers in the human body that go on to create the sensation of nausea and vomiting:

The central nervous system (CNS)

Stimuli can affect areas of the CNS including the cerebral cortex and the limbic system. These areas are activated by elevated intracranial pressure, irritation of the meninges i.e. blood or infection, and extreme emotional triggers such as anxiety. The supratentorial region is also responsible for the sensation of nausea.

Chemoreceptor trigger zone (CTZ)

The CTZ is located in the area postrema on the floor of the fourth ventricle within the brain. Common triggers of the CTZ include metabolic abnormalities, toxins, and medications.

Vestibular system

These include movements that cause motion sickness and dizziness. 

Peripheral pathways

These pathways are triggered via chemoreceptors and mechanoreceptors in the gastrointestinal tract, as well as other organs such as the heart and kidneys. Common activators of these pathways include toxins present in the gastrointestinal lumen and distension of the gastrointestinal lumen from blockage or dysmotility of the bowels. Signals from these pathways travel via multiple neural tracts including the vagus, glossopharyngeal, splanchnic, and sympathetic nerves.

Pre-nausea pathophysiology

It has been described that alterations in heart rate can occur as well as the release of vasopressin from the posterior pituitary.

What are the best treatments for Nausea?

If dehydration is present due to loss of fluids from severe vomiting, rehydration with oral electrolyte solutions is preferred. If this is not effective or possible, intravenous rehydration may be required. Medical care is recommended if:

  • A person cannot keep any liquids down
  • Has symptoms more than 2 days
  • Is weak
  • Has a fever
  • Has stomach pain
  • Vomits more than two times in a day, or
  • Do not urinate for more than 8 hours.

Nausea Medications

Many pharmacologic medications are available for the treatment of nausea. There is no medication that is clearly superior to other medications for all cases of nausea. The choice of antiemetic medication may be based on the situation during which the person experiences nausea.

For people with motion sickness and vertigo, antihistamines and anticholinergics such as meclizine and scopolamine are particularly effective.

Nausea and vomiting associated with migraine headaches respond best to dopamine antagonists such as metoclopramide, prochlorperazine, and chlorpromazine. 

In cases of gastroenteritis, serotonin antagonists such as ondansetron were found to suppress nausea and vomiting, as well as reduce the need for IV fluid resuscitation. 

The combination of pyridoxine and doxylamine is the first-line treatment for pregnancy-related nausea and vomiting. 

Dimenhydrinate is an inexpensive and effective over-the-counter medication for preventing postoperative nausea and vomiting. 

Other factors to consider when choosing an antiemetic medication include the person’s preference, side-effect profile, and cost. Nabilone is also indicated for this purpose.

Alternative medicine

In certain people, cannabinoids may be effective in reducing chemotherapy-associated nausea and vomiting. Several studies have demonstrated the therapeutic effects of cannabinoids for nausea and vomiting in the advanced stages of illnesses such as cancer and AIDS.

Ginger has also been shown to be potentially effective in treating several types of nausea.

What are the best Home remedies for Nausea? 

There are many anti-nausea medications that can help with severe or persistent nausea. But if your nausea is mild or occasional, there are also a number of effective home remedies that can help relieve your symptoms naturally:

1. Ginger 

Ginger is an effective remedy for nausea, says Daniel Devine, MD, internal medicine doctor, and geriatrician at Devine Concierge Medicine, a primary care practice in Philadelphia. 

A 2014 analysis of six different studies published in the Journal of the American Board of Family Medicine examining the use of ginger in pregnancy found that taking about one gram of ginger once a day for at least five days decreased symptoms of nausea and vomiting in early pregnancy. Studies have also found that ginger can be effective in managing nausea and vomiting symptoms for chemotherapy patients. 

You can also add pieces of whole, fresh ginger to your tea, or include it as a spice or seasoning in your food. 

2. Peppermint

Peppermint has long been regarded as a traditional remedy for nausea, though the scientific evidence on its efficacy is not as robust as it is for ginger, Devine says. Still, many people swear by its calming properties. 

One small study from 2014 published in the Journal of Perianesthesia Nursing suggested that even the scent of peppermint oil can alleviate nausea, but more research is needed to determine whether it is an effective remedy. 

If you want to give peppermint a try, you can buy it as a tea or diffuse peppermint essential oil for aromatherapy by adding two to three drops of peppermint oil to a diffuser filled with water.

3. Small, bland meals

Eating too much can cause nausea, Devine says. That’s because when you eat too much, it stretches the stomach, resulting in bloating, heartburn, and excessive digestive movement — all of which can lead to nausea.  

Eating small, frequent meals and consuming a bland diet without strong flavors can be helpful in reducing episodes of nausea, Devine says. Bland foods are easy to digest and can help settle your stomach. 

Bland foods that can help with nausea include:

  • White bread or toast
  • Plain chicken 
  • Rice
  • Applesauce
  • Soup
  • Bananas
  • Saltine crackers

If you’re feeling queasy, you should avoid spicy food and acidic beverages like soda, juice, or alcohol — all of which can exacerbate nausea symptoms. You may even want to consider trying the BRAT diet when you feel nauseous. 

4. Stay hydrated 

It may be hard to eat or drink anything when you have nausea — including water. But according to Devine, dehydration will only make your nausea worse. 

This can be especially important if you’re experiencing nausea as a result of extreme heat or humidity. In fact, nausea and vomiting are some of the main symptoms of heat exhaustion and heatstroke. 

Overheating causes your blood vessels to dilate as your body tries to cool itself down — and this change in blood pressure can manifest as nausea or dizziness. But if you drink lots of water and stay hydrated, it will help you cool down and return to a normal body temperature. 

If drinking water is a challenge for you with nausea, you should take small sips throughout the day or try a soothing beverage like warm peppermint tea.

5. Sit upright 

Lying flat while nauseous could lead to vomiting, Devine says. “It is important to use gravity to your advantage and keep your head inclined above your stomach,” Devine says. By staying upright, gravity helps keep your stomach contents down. 

6. Acupressure 

Acupressure is an alternative medicine practice of applying pressure to certain points on the body, known as meridians. The idea is that by putting pressure on these places, you send a message to the body to turn on its self-healing mechanisms, which may alleviate pain or nausea. 

A 2006 review of more than 40 trials published in the journal Autonomic Neuroscience found that acupressure can reduce some symptoms of nausea.

1. Siddha preventive measures

Everybody must practice Siddha preventive measures, whether a person is affected with nausea or not, but preventive measures are the primary steps for switching on to any other Siddha remedies, and hence they are important. It helps in one’s capability, effectiveness, and productivity, decision making power, intellectuality, and removing minor health problems. There are three types of preventive measures:

  • Earthing – performed for earthing the negativity of our body

Severe, untreated salt imbalance can result in “extreme weakness, confusion, coma, or death.” The diet was first discussed in 1926.

Nutritional analysis of BRAT diet

The American Academy of Pediatrics states that most children should continue a normal, age-appropriate diet. BRAT diet is no longer routinely recommended to those who have had stem cell transplants and have diarrhea due to graft-versus-host disease, as long-term use can lead to nutritional deficiencies.

Adding rice, bananas, or pectin to the diet while suffering from diarrhea may be beneficial, but physicians Debora Duro and Christopher Duggan point out that the diet is not nutritionally complete and may be deficient in energy, fat, protein, fiber, vitamin A, vitamin B12, and calcium. Duro and Duggan also say that food restriction does not alleviate diarrhea and actually causes individuals to have diarrhea for longer periods, according to randomized clinical trials.

Medical attention is required when on the BRAT diet if any blood or mucus is present in diarrhea, if the DIARRHEA is severe, or if it lasts longer than 3 days.
