Plaque

Tongue scrapers

The tongue contains numerous bacteria which causes bad breath. Tongue cleaners are designed to remove the debris built upon the tongue. Sugars are commonly associated with dental cavities. Other carbohydrates, especially cooked starches, e.g. crisps/potato chips, may also damage teeth, although to a lesser degree (and indirectly). Because starch has to be converted to glucose by salivary amylase (an enzyme in the saliva) first. Sugars that are higher in the stickiness index, such as toffee, are likely to cause more damage to teeth than those that are lower in the stickiness index, such as certain forms of chocolate or most fruits.

Sucrose (table sugar) is most commonly associated with cavities. The amount of sugar consumed at any one time is less important than how often food and drinks that contain sugar are consumed. The more frequently sugars are consumed, the greater the time during which the tooth is exposed to low pH levels, at which point demineralization occurs (below 5.5 for most people).

Sucrose is used by Streptococcus mutans bacteria to produce biofilm. The sucrose is split by glucansucrase. It allows the bacteria to use the resulting glucose for building glucan polymer film resulting fructose as fuel to be converted to lactic acid.

Acids contained in fruit juice, vinegar, and soft drinks lower the pH level of the oral cavity which causes the enamel to demineralize. Drinking drinks such as orange juice or cola throughout the day raise the risk of dental cavities tremendously.

Another factor which affects the risk of developing cavities is the stickiness of foods. Some foods or sweets may stick to the teeth and thus reduce the pH in the mouth for an extended time. If they are sugary in particular.

Mouthwash

There are three commonly used kinds of mouthwash. These are:

Saline

Saline (warm salty water) is usually recommended after procedures like dental extractions. In a study completed in 2014, warm saline mouthrinse was compared to no mouthrinse in preventing alveolar osteitis (dry socket) after extraction. 

Essential oils (EO) or cetyl pyridinium chloride (CPC)

Essential oils, found in Listerine mouthwash, contains eucalyptol, menthol, thymol, and methyl salicylate. CPC containing mouthwash contains cetyl pyridinium chloride, found in brands such as Colgate Plax, Crest Pro-Health, Oral B Pro-Health Rinse. 

Chlorhexidine

Chlorhexidine gluconate is an antiseptic mouthrinse that should only be used in two-week time periods due to brown staining on the teeth and tongue. Compared to essential oils, it is more efficacious in controlling plaque levels but has no better effect on gingivitis.

Sodium hypochlorite

As mentioned earlier, sodium hypochlorite, a common household bleach. It can be used as a 0.2% solution for 30 seconds two or three times a week as a cheap and effective means of combating harmful bacteria. 

DENTURE CARE

Dentures, retainers, and other appliances must be kept extremely clean. It is recommended that they are cleaned mechanically twice a day with a soft-bristled brush and denture cleansing paste. It is not recommended to use toothpaste.

Dentures should be taken out at night. Leaving them in whilst sleeping has been linked to poor oral health. Leaving a denture in during sleep reduces the protective cleansing. Antibacterial properties of saliva against Candida albicans (oral thrush) and denture stomatitis.

Keeping dentures dry for 8 hours significantly reduces the amount of Candida albicans on an acrylic denture. It is recommended to soak a denture overnight minimum once in a week with an alkaline-peroxide denture cleansing tablet, as this has been proved to reduce bacterial mass and pathogenicity.

Oral cancer is a type of head and neck cancer. It is often treated similarly to other head and neck cancers. This cancer mostly happens after the age of 40, and the risk is more than twice as high in men as it is in women.

Symptoms

Oral cancer can appear as a lesion or tumor anywhere in the mouth. In the early stages, there are often no signs or symptoms, but smokers and heavy drinkers should have regular checkups with the dentist, as they may identify early signs. The signs and symptoms include:
  • Patches on the lining of the mouth or tongue, usually red or red and white in color

  • Swelling that persists for over 3 weeks

MEDICAL SCIENCE – TREATMENT

Treatment depends on the location and stage of cancer, and the patient’s general health and personal preferences. A combination of treatments may be necessary.

Surgery

Surgical removal of the tumor involves taking out the tumor and a margin of healthy tissue around it. A small tumor will require minor surgery, but for larger tumors, surgery may involve removing some of the tongue or the jawbone.

If surgery significantly changes the appearance of the face, or the patient’s ability to talk, eat, or both, reconstructive surgery may be necessary. This involves using transplanted grafts of skin, muscle, or bone from other parts of the body.

Oral cancers are especially sensitive to radiation therapy, which uses beams of high-energy X-rays or radiation particles to damage the DNA inside the tumor cells, destroying their ability to reproduce.

Brachytherapy is often used to treat patients with early stages of cancer of the tongue. Radioactive wires or needles are stuck directly into the tumor, releasing a dose of radiation into the tumor. The patient is usually under general anesthetic. A course normally lasts from 1-8 days.

A person with early-stage mouth cancer may only need radiation therapy, but it can also be combined with surgery, chemotherapy, or both, to prevent cancer from returning. Radiation therapy may remove cancer if a patient is in the early stages of oral cancer.

Chemotherapy

It is radiotherapy, chemotherapy and surgery are three treatment options. Widespread cancer may be treated with chemotherapy as well as radiation therapy, especially if there is a significant chance of the cancer returning. It involves using powerful medicines that damage the DNA of the cancer cells, undermining their ability to reproduce.

Causes

Cancer results when a genetic mutation instructs cells to grow without control. Untreated, oral cancer will start in one part of the mouth, then spread to other parts of the mouth. Gradually to the head and neck, and the rest of the body. Mouth cancers typically start in the squamous cells that line the lips and the inside of the mouth. This is called squamous cell carcinoma.

The exact cause of the mutations is unknown, but there is evidence that certain risk factors increase the chance of mouth cancer developing.

Risk factors

Tobacco and alcohol use are prominent risk factors for mouth cancer. Smokers are three times more likely than nonsmokers to develop mouth cancer. People who smoke and drink alcohol have up to 30 times higher risk than those who do not smoke and drink. The other risk factors include:
  • Ultraviolet exposure on the lips, from the sun, sunlamps, or sunbeds
  • A diet rich in red meat, processed meat, and fried foods
  • Gastro-esophageal reflux disease (GERD), a digestive condition where acid from the stomach leaks back up to the gullet
  • Human papillomavirus (HPV) infection
  • Prior radiation treatment, or radiotherapy, in the head, neck, or both
  • Regularly chewing betel nuts, a popular habit in some parts of Southeast Asia
  • Exposure to certain chemicals, especially asbestos, sulfuric acid, and formaldehyde

Diagnosis

The dentist may be the first person to spot signs of mouth cancer. A biopsy may be carried out, where a small sample of tissue is taken to check for cancerous cells. Sometimes a “brush biopsy” is used initially where cells are painlessly collected by brushing them to one side.

If mouth cancer is diagnosed, the next task is to determine the stage of cancer. Tests include:

  • Endoscopy, where a lighted scope is passed down the patient’s throat to see how far cancer has spread.
  • Imaging tests, such as X-ray, Computed Tomography (CT) scans, and Magnetic Resonance Imaging (MRI) Staging cancer will inform treatment options and help predict prognosis.

A person who has been diagnosed with stage 1 oral or pharyngeal cancer has an almost 83 percent chance of surviving for longer than 5 years. Whose cancer spreads to other parts of the body, the chance of surviving more than 5 years is 38 percent.

  • Croup – inflammation, usually in small children, which causes a barking cough
  • Laryngitis – swelling of the voice box, which can cause a hoarse voice or to the extent of ‘loss of voice’

Most throat problems are minor and go away on their own. Treatments, when needed, depending on the problem.

Symptoms of Throat

One may have the following symptoms, which may be treated as early as possible:-

  • Voice changes like cracking or hoarseness
  • Trouble swallowing or breathing
  • A sore throat, cough, or an earache that won’t go away
  • A headache
  • A neck lump
  • Unexplained weight loss

Instruction: See your doctor right away if any symptoms last for more than a few weeks.

Causes of Throat 

Using tobacco for a long time. Smoking it and chewing it are the biggest risk factors for all head and neck cancers, including throat cancer. Drinking heavily and regularly. That means more than two drinks of alcohol a day if you’re a man or more than one day if you’re a woman.

Diagnosis of Throat

A sore throat is usually from irritation or inflammation. The most common cause (80%) is acute viral pharyngitis, a viral infection of the throat.

Other causes include other infections (such as streptococcal pharyngitis), trauma, and tumors. Gastroesophageal (acid) reflux disease can cause stomach acid to back up into the throat and also cause the throat to become sore. In children, streptococcal pharyngitis is the cause of 37% of sore throats.

It is usually/mostly caused by pharyngitis (inflammation of the throat) or tonsillitis (inflammation of the tonsils). It can also result from trauma. About 7.5% of people have a sore throat in any three-month period.

Throat irritation can refer to a dry cough, a scratchy feeling at the back of the throat, a sensation of a lumpy feeling, something stuck at the back of the throat, or possibly a feeling of dust in the throat. The symptoms are unpleasant and usually temporary, but occasionally signifies a more serious health issue, such as laryngitis.

Throat – Allergies: During the summer months, allergies are a common cause of throat irritation. Many individuals have allergies to pet dander, dust, mitespollen, and molds. They can trigger an allergic reaction which presents with a runny nose, red eyes, congested nose, and throat irritation. Often a dry cough may also be present.

Throat – Laryngitis

It is inflammation of the voice box which can occur from overuse, irritation or an infection. Laryngitis can be a short-term illness or a prolonged problem. The majority of cases of laryngitis are due to viral infections which only last a few days.

Laryngitis is often a common complaint in individuals who sing. Opera singers or those who yell at sporting events strain the throat muscles and develop a case of laryngitis. However, persistent throat irritation and hoarseness may be from a more serious disorder like cancer.

Throat – Pharyngitis

Viruses are common causes of the common cold. Less often, bacteria may also cause pharyngitis. Both of these organisms enter the body via the nose or mouth as aerosolized particles when someone sneezes or coughs. Because many germs are contagious, one can even acquire them from touching utensils, toys, personal care products or door knobs.

The most common viruses that cause throat irritation include the common cold virus, influenza, infectious mononucleosis, measles, and croup. Most bacteria and viruses usually induce throat irritation during the winter or autumn.

Throat – Epiglottitis

It is a very serious disorder of the back of the throat near the windpipe. The most common cause of epiglottitis is an infection by the bacteria, H influenza.

The condition may present all of a sudden with high fever, severe sore throat, difficult and painful swallowing, drooling saliva, hoarse voice, difficulty breathing and malaise. The condition is life-threatening and needs immediate hospitalization.

Epiglottitis is treated with antibiotics. Routine vaccination has made epiglottitis very rare but it still does present in some children. Prompt diagnosis and treatment can be life-saving.

Throat – Post-nasal Drip

Also called rhinorrhea, is a very common medical disorder that occurs when the nasal tissues are congested and the excess fluid runs either at the back of the throat or out of the nose.

Post-nasal drip can be caused by the common cold, allergies to dust, smoking, or pet dander. Even spicy foods can sometimes cause post-nasal drip. A runny nose is not life-threatening but can be uncomfortable and socially unacceptable.

Strep Throat

It is caused by bacteria which if untreated can lead to many other problems in the body. Strep throat is most common in childhood but can affect people of all ages.

It may present with throat pain, difficulty in swallowing food etc, painful and swollen tonsilsfeverheadache, skin rash, and flu. The diagnosis of strep throat is straightforward and the treatment requires a course of penicillin.

However, if the treatment is not adequate, rheumatic fever can occur with resultant damage to the heart valves.

Treatment – Medical Science

The majority of cases of throat irritation usually go away without any treatment. There is no real treatment for throat irritation from a virus. If you have difficulty swallowing then one should drink liquids, suck on lozenges, ice chips or mix salt with warm water to gargle. Bacterial infections generally require antibiotics for its cure.

Home remedies for throat irritation include gargling with warm water twice a day, sipping honey and lemon mixture or sucking on medicated lozenges.

If the cause is dry air, then one should humidify the home. Since smoke irritates the throat, stop smoking and avoid all fumes from chemicals, paints and volatile liquids. Rest your voice if you have been screaming or singing.

Treatments – Ayurveda

The diagnosis of diseases involves identifying its causes. Identification of causative factors is through the examination of pulse, urine, eyes, the study of voice, the color of body, tongue and the status of the digestive system.

It holistic in approach and the diagnosis involves the study of a person as a whole as well as his disease. If you have pharyngitis, avoid infecting others by covering your mouth when coughing and wear a mask.

1. How To Solve Throat Problems: ‘Siddha Shaktidata Yog’

Where there is a problem (in this case, we have a problem with the throat) in the body at a particular spot, where the energy should be rotated for little longer time. Finish this process within 3 minutes.

Open the eyes by leaving the energy at the throat having a feeling that the energy is going through inside mouth to complete throat, veins, and surrounding. Then open the eyes slowly.
